In order to opt out of the Arbitration Agreement and Jury-Trial and Class-Action Waivers, you must complete the Arbitration Opt-Out Form below within 90 calendar days of the date you purchased or otherwise took ownership or other interest in your Windsor Products.

You will then receive an email confirming your email address.
Upon confirmation of your email address your form will be submitted to Windsor Windows for our records.
